Business Visa

• Visa form to be filled online at www.mumbai.mfa.gov.pl and has to take an appointment for submission and needs to come personally for submission.
• Original Passport valid for 6 months and also require passport copy 1st and last page 2 sets
• 2 Recent Photographs 35mm x 45mm with face 80% white background & without border (Photographs should not be old for more than 6 months)
• Covering Letter from Company
• Invitation Letter with proper details, address and contact number and also the person who signs, his name, designation and email is to be mentioned.
• Return ticket
• Bank Statement with Original Bank Seal for the last 6 months
• Pay slips for the last 3 months
• Income tax papers for the last 3 years
• Copy of the PAN Card
• Medical Policy
• Hotel confirmation

Tourist Visa

• Visa form to be filled online at www.mumbai.mfa.gov.pl and has to take an appointment for submission and needs to come personally for submission.
• Original Passport valid for 6 months and also require passport copy 1st and last page 2 sets
• 2 Recent Photographs 35mm x 45mm with face 80% white background & without border (Photographs should not be old for more than 6 months)
• Detailed Personal Covering letter
• Leave letter from Company
• Hotel Confirmation
• Return ticket
• Bank Statement with Original Bank Seal for the last 6 months
• Salary slip for the last 3 months
• Income tax papers for the last 3 years
• Copy of the PAN Card
• Medical Policy

All the documents should show travel date after 8 to 10 days.
